ESLHO tries to continuously improve the ESLHO EQA Program, including the ESLHO EQA Portal. If you have a complaint, please submit it using the form below.
After successful submission of your complaint, you will see a confirmation message in your browser.
If you enter your email address, we will send you a confirmation by email when the complaint was received by one of our colleagues. We will then assess the complaint and might request additional information if this is needed. We will try to resolve your complaint as quickly as possible and will provide you with updates on progress or outcome.
